A detailed prescription from a Pulmonologist/ and or your sleep doctor ordering a CPAP/ and or BIPAP machine with the perscribed pressure and supplies
A copy of your Sleep Study and supporting physician notes

Going out of pocket

We offer out of pocket pricing on select machines; as well as refurbished machines. Pricing ranges from $750 - $1,150.
